Initial each statement and sign below, by doing this you agree to these team requirements:
_____1.) Swimmer will be a member of the USA Swimming (membership fees included in registration).

_____2.) PAYMENTS PLANS – One half of the total fees are due at time of registration, balance due by November 1st OR One third of total fees due at time of registration, 2nd payment (1/3) of fees due OCT. !st, balance due on November 1st. Late fees will be imposed on unpaid balance. Varsity Rates will not be available after November 1st.

_____3.) Swimmer’s attendance is MANDATORY for a min. 6 meets. (2 Home and 4 Away)

_____4.) Parent(s) Attendance is MANDATORY for the Parents Meeting (Tues, 9/21 or Wed, 9/22)

_____5.) U.S. meet individual event fees are due prior to the date of the meet. $5 per swimmer surcharge for each meet will be due with entry fees. Relay entry fees will be paid by VAC.

_____6.) Swimmers will attend at least (3) three practices/week and arrive on time and prepared for practice. No Swimmer under the age of 11 can be dropped off, a parent must stay or Swimmer(s) will not be allowed to swim.

 _____7.) Team suits and cap are the required uniform at all meets. Each swimmer will abide by rules and regulations outlined in the VAC Handbook. (Available on Web site – vacswim.com)

_____8.) I, the undersigned, and agree to volunteer for a TOTAL of (5) Sessions =(4) Sessions at The Viking Classic Meet (11/12 – 11/14/10) & (1) Session at the Quick Freeze Meet (1/22/11), even if my child is not scheduled to swim in the meet. All Parents must sign up . for a Committee & Position and will be required to assist in Team & Meet Management.

_____9.) $100 Work Bond will be Collected at Registration for each family to insure volunteer Sessions (5) are completed. If all (5)sessions are not done, check will be cashed.

____10.) Refunds will be given during the first 2 weeks of the season only, less a $57.00 Non-refundable USA Swim fee/swimmer. You must cancel by contacting board member Jenn Krzeminski at jkginda@yahoo.com within the first 2 weeks of the season. Acknowledgement must be given by Jenn for refund.

_____11.) **Varsity rates are only available if swimmer is registered and paid by November 1, 2010. Competitive Rates apply to all registrations received after Nov. 1st. Varsity girls may cancel Up to 2 weeks after the Section 9 Swim Finals, less a $57.00 non refundable USA Registration Fee.
